| At your first visit, we will ask you to complete a patient information form. This form will ask you to include your medical and dental records and insurance information. | |||||||||
| Dr. Prater will conduct a complete orthodontic clinical exam. She will discuss with you any problems that are discovered and describe the necessary corrective treatment. | |||||||||

| You will also receive an approximate fee and estimate of treatment time. There is no charge for your initial examinations. For patients whose growth and development need to be monitored prior to treatment, there is no charge for periodic appointment supervision. | |||||||||
